M-TH: 8AM-9PMÂ
Fri: 8AM-5PMÂ
Sat: 8:30AM-NOONÂ
Walk in Lab: Mon-Fri: 6-9AMÂ
Work Schedules: Shifts are varied. Saturday shifts are schedule by rotation.
JOB SUMMARY :
The Licensed Practical Nurse, LPN will provide direct and individualized nursing care to an assigned group of patients under the direction of a registered nurse of provider. The LPN will observe, report and record medical information and nursing care provided in the electronic medical record. The LPN will instruct patients and families according to the teaching plans. The LPN will communicate with the healthcare team to facilitate the coordination of individualized patient care.
MINIMUM QUALIFICATION(S):
LPN License to practice in the state of employment or eligible for licensure in state of employment required.
Basic Life Support (BLS) required prior to completion of orientation.
PREFERRED QUALIFICATION(S):
One (1) year experience in an acute or ambulatory care setting preferred.Â
PHYSICAL REQUIREMENTS:Â
Able to stand & sit constantly; walk frequently.Â
Able to lift/carry 0-10 lbs. frequently; 11-50 lbs. occasionally.Â
Able to push/pull 0-20 lbs. frequently; 21-75 lbs. occasionally.
Able to climb occasionally; reach constantly.Â
Able to use hands for simple/firm grasping.Â
