MultiCare Health System seeks a 0.80 or 1.0 FTE experienced Physician Assistant or Nurse Practitioner to join the Care Continuum team.The APP-Care Continuum is responsible for providing comprehensive care of elders in either an ambulatory or SNF setting. Ambulatory or transitional care in post-acute and long-term care facilities of vulnerable adults and/or elders living in the community or requiring post-acute care. This position is an independent clinician which works on a collaborative basis with other health care providers. Work situations are varied and require discretion and good judgment in decision making. This position works with all socioeconomic levels in the community.
