ChristianaCare is looking for a Patient Safety Program Manager to join its team The Program Manager will for monitoring and ongoing refinement of ambulatory safety and compliance to Joint Commission accreditation standards across all Medical Group of Christiana Care (MGCC) office practices and extension with community partners. Responsible for identifying opportunities to improve patient safety. Facilitates and successfully implements an action plan to achieve success in safety and regulatory improvement initiatives.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Facilitate performance improvement initiatives as appropriate. Facilitate processes to continually evaluate safety information. Monitor established safety results and share results with leadership and recommendations for actions as needed.
Participates on the Ambulatory Clinical Effectiveness Committee as needed Serve as the content expert and facilitator for safety and accreditation related discussions at the Ambulatory Safety Committee and Event Reporting Teams meetings. Participate in the development of safety metrics.
Develop a system for monitoring and improving patient and caregiver safety. Identify trends including opportunities for improvement and celebration. Monitor and evaluate the effectiveness of MGCC clinical performance improvement processes to improve the quality and safety of patient care and outcomes.
Develop and maintain documentation of root-cause analysis and risk-reduction strategies and reports as required.
Coordinate dissemination of information and assesses organizational compliance related to regulatory bodies or events such as National Committee for Quality Assurance, National Patient Safety Goals, Sentinel Event Alerts and recommendations as directed by MGCC leadership. Participates in ambulatory practice rounds for the purposes of surveying to assess compliance with environment of care, life safety and TJC accreditation standards and provides feedback to local site and leadership for action planning.
Participate in ongoing compliance assessment activities in partnership with practices and ambulatory services. Identifies potential compliance issues based on system rounds and tracer activities. Assist in Joint Commission survey, CMS survey and other regulatory surveys in partnership with departmental leadership.
Assist with preparation of action plans in response to accrediting organization and State/CMS surveys. Develops and establishes organizational policy, procedures and processes as needed for regulatory and accreditation requirements to assure organizational compliance. Makes recommendations regarding PI process, TJC accreditation standards and other regulatory agency requirements as identified.
Provides ongoing education regarding TJC standards, intent, scoring and compliance. Maintain current knowledge of TJC and other regulatory standards and Performance Improvement methodologies (ie FOCUS-PDCA).
Performs assigned work safely, adhering to established departmental safety rules and practices; reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.
Performs other related duties as required.

EDUCATION AND EXPERIENCE REQUIREMENTS:
Must have BSN and be Licensed by State of DE as an RN, Master's preferred
Three to five years of clinical nursing experience in ambulatory practice, medical-surgical, emergency department or critical care experience
Proven leadership skills and behaviors
Organizational skills and project management experience required.
